Medical Nutrition Therapy

What it is

Medical nutrition therapy (MNT) is a dietary treatment for chronic and short-term health conditions. Registered dietitians (RD) are the only professionals that can provide MNT. Your dietitian, Kelly, will counsel you on a therapeutic diet. The first step is a nutrition assessment. Together, you will decide on a nutrition intervention and then monitor your progress.

You can see Kelly Cornelius, RD for medical nutrition therapy with a doctor’s referral for conditions including, but not limited to: diabetes, heart disease, high cholesterol, high blood pressure, kidney disease; gastrointestinal problems such as acid reflux, irritable bowel syndrome, gallbladder disease.

How to get started

Talk to your healthcare provider about seeing a dietitian. California law requires a written referral/prescription (Rx). The Rx should include your diagnosis (with ICD-10 code) and your goal for medical nutrition therapy. And of course, basic patient identification (name, date of birth, contact) plus your provider’s information (name, contact, National Provider Identification #). This form can be sent securely to Kelly Cornelius, RD. You will be contacted to set up an appointment.

How to pay for services

I, Kelly Cornelius, RD, am committed to helping all people who need medical nutrition therapy. That’s why I am working toward contracting with health insurance companies to make services affordable to you. In the meantime, you can check with your insurance company for out-of-network benefits. You may qualify for reimbursement. See “pricing” for out-of-pocket costs.
